Description
Orders from Canada must be accompanied by a prescription from a doctor. The prescription must show the doctor’s registration number, address, telephone number as well as the doctor’s signature on the prescription.
For customs clearance purposes we also require a copy of the details page of the person’s passport. A copy of the person’s work permit or study visa as well as a letter of intent needs to be provided.
These documents must be on separate uploads in pdf format.
All orders to Canada may not be for a quantity greater than 3 months supply.
